IMPORTANT DIVING INFORMATION
Malta Tourism Authority rules state that all divers must complete a Self Declaration Medical Form before diving. Please download your Self Declaration Medical Form here.
If you do have any of the medical conditions listed, signed clearance from a doctor will be required.
You will need to complete this and hand it to the dive centre or dive guide before diving.
